NO PURCHASE NECESSARY. To enter, fill out this form or pick up write-in entry form from Brother Sebastian’s Steakhouse and Winery, 1350 S. 119^th St.. Incomplete entries will be disqualified. All entries become the property of Brother Sebastian’s Inc.. Brother Sebastian’s Inc. is not responsible for lost, misdirected or delayed entries.

CONTEST RULES

1. The contest is open to all adults 21 or older. Identification must be produced upon request. Employees of Brother Sebastian’s, Inc., Clear Channel Broadcasting, Inc., Old Market Limousine, Inc., and relatives are excluded from participating in this contest.

2. No prizes nor portions thereof may be redeemed for cash and must be accepted as awarded.

3. Prizes to be awarded:

One $100 gift certificate to Brother Sebastian’s. Winner to be determined by random drawing on three Mondays in April, 2007 (9^th, 16^th, and 30^th). All entrants will be eligible.

One "Great Date" Grand Prize, comprised of all inclusive dinner for two, tickets to see Billy Joel live at the Omaha Qwest Center, and limousine transportation (provided by Old Market Limousine Service). Winner to be determined by random drawing on Monday, April 23^rd, 2007. All entrants will be eligible.

4. Entries will be drawn at random for weekly prizes on each Monday in April, beginning Monday, April 9^th. The "Great Date" Grand Prize will be awarded Monday, April 23^rd. "Favorite Moments" Contest entries MUST be received Saturday, April 21^st, 2007. "Favorite Moments" entries received after April 21^st, 2007 will be disqualified. All prizes will be announced by Gary Sadlemyer, on KFAB’s "The Morning Show" each Monday morning. Winners may also be contacted by phone or email and informed of acceptance details.

5. In accepting the prize, the Great Date Grand Prize winner acknowledges that Brother Sebastian’s, Inc., Clear Channel Broadcast, Inc., and Old Market Limousine, Inc. may not be held liable for any loss, injury or damages associated with accepting or using this prize(s).

6. Limousine service will be provided by Old Market Limousine, Inc. Old Market Limousine, Inc. agrees to provide transportation only from winners home to Brother Sebastian’s Steakhouse and Winery, then to Qwest Center Omaha and home again. Home pickup and delivery will be provided within 10 miles of Omaha city limits only. In the event the winner lives outside the limited area a pre-arranged pickup/dropoff point will be determined.

7. By claiming the prize the winner authorizes the use, without additional compensation of his or her name and/or voice/photograph, the written entry or any portions thereof for promotion and/or advertising purposes in any manner and in any medium (including, without limitation, radio broadcasts, newspapers and other publications, and internet) which Brother Sebastian’s, Inc. may deem appropriate.
